Insurance
We accept most PPO Health Care plans (including Anthem Blue Cross), Worker's Compensation cases, and Medicare.

Scheduling
Please contact our office at (949) 429-2155 to schedule your appointments.  As a courtesy to you, you will be asked to provide your insurance information so that we can inform you of your benefits prior to your appointment.

Your First Visit
Please arrive 10 minutes before your scheduled appointment time.  There will be some paperwork to complete prior to your evaluation and teatment. You can call or email our office to request your paper work via fax or email prior to your appointment.

What to Bring
Please bring your driver's license, insurance card, and the physical therapy referral you received from your physician.  Your physician may fax the referral to our office at (949) 429-2151.

In addition, bring comfortable clothes for treatment and/or exercise. Treatment gowns are provided for treatment as needed.
