We accept most PPO Health Care plans, Worker's Compensation cases, and Medicare.

Scheduling
Please contact our office at (949) 429-2155 to schedule your appointments.  As a courtesy to you, you will be asked to provide your insurance information so that we can inform you of your benefits prior to your appointment.

Your First Visit
Please arrive 10 minutes before your scheduled appointment time.  There will be some paperwork that will need to be completed prior to your evaluation and teatment. You can call or email our office to request your paper work via fax or email prior to your appointment.

What to Bring
Please bring your driver's license, insurance card, and the physical therapy referral your received from your physician.  Your physician may fax the referral to our office at (949) 429-2151.
